Immunizations
Submit immunizations if you have not done so – you will not be able to attend classes until this is submitted.
Vaccines are available at the CSU Health Clinic for Hepatitis B, Tetanus, Diphtheria and Pertussis (Tdap). The flu shot is available at certain times that will be announced. We can also test for tuberculosis. All are available on request at reduced cost.
Please review both pages of this form carefully. You may not need all of the vaccines based on your age.
If you need the MMR (measles, mumps, rubella) upon arrival they will be available our local Columbus Department of Public Health Center or through local health doctor's or care centers. The Public Health Center would be the most cost effective option.
