Fillable forms are completed best on your computer with free Adobe software. If they are completed on your phone, they may not fill correctly. Fillable forms may be attached to an email and sent to student_health_services@columbusstate.edu. If you are not able to use the fillable option, please print the forms, complete them, and drop them off at the SHC prior to your appointment.
